Although you want a flat rate most design agencies do work on an hourly rate. The reasons are many but mainly you cannot predict the client’s reaction to your designs, you might have to produce several visuals before you get one accepted, then you create an artwork, you will probably then have a series of revisions before it is signed off.
You could, by predicting how long it will take you to do several visuals, attend meeting(s) with the label and/or band, possibly art direct photography, then produce an artwork create a flat rate fee then additionally have hourly rates for the revision stages.
The linked pdf might give you some thoughts on how to calculate the prices.
